Seriously though, maybe the problem with not being able to focus, is that forcing your brain to become more focused intensifies the problem?
Could not being able to focus, be a sign that you need a break / change / something completely different?

I swear by my Moleskine daily journal. I make a list,organize it according to the hours of the day. Always get more than I expected done.

Leave QBN. Close all web windows unless you're actually intentionally looking for things online. Stay off of Facebook, Twitter, etc. Don't set your email reminder to ding every time to get a new message.
I'm working on taking all the above advice... starting... now.

Over time, working in isolation (even if you function as part of a team) can prove harmful to your mental health, business and website. In fact, even if other people are working on a project of yours, if they are junior to you, you can still feel isolated. http://www.smashingmagazine.com/…

what ever is distracting you make it harder to reach, TV unplug it , web surfing, unplug modem , bunker down , also helps to tell people what you are working on or just tell people you plan on having X project done by X date. Set a goal to drive you , people will slowly take interest and ask you about how your projects are going

take a walk or bike ride every morning at the same time...
every 30 minutes or so, look out the window so your eyes don't get all computered-out
do NOT listen to talk radio
do NOT have a hundred browser windows open reading random shit
I use a timer.... say 20 minutes to do x, then I can do y.. strangely I get alot done in short bursts without random inturruptions of people that happen in an office
