    What I've done is use Illustrator. I scan my signature and then save it out as a JPEG with a white background, tweak the brightness etc. Then I open the PDF page in Illustrator, place the signature, scale it appropriately, and then set its blend mode to multiply. Then fill out the rest of the stuff using the type tool.

    The new Preview in Lion has signature support, which is essentially the same thing as above. However, if the PDF wasn't made correctly to type into for the other stuff, it prolly makes sense to just do it in Illy.

    There's also a digital signature in PDF but some folks might think it's not going to pass legal (when it actually does)
