Dressing as a "Freudian Slip" is the #1 costume used by corporate schmucks at a Halloween party, who think they are clever by thinking of it.
2 most common executions:
Female wearing a "slip" with a suggested phrase taped to it.
Male or female dressed in black with post it notes all over with suggestive phrases written on them.
