For something complicated like this I usually create a mockup to the best of my ability and discuss it with the printer. I take their suggestions where possible to make their life easier and get a great end result.

Do you have to send them complex diagrams, or do you just send a file with notes?

Nice. Not as complicated as it looks. Only things I notice are the embossing, vellum, and im guessing the "11" tab is the month. Most printers have embossing capabilities but other than that its just choosing the paper and size. If you show this to your local printer they may have a similar sample in their portfolio already.
